this is when our entire dance community

comes together to celebrate the performing arts

This module is part of our syllabus extension coaching dance education stream

and is available to all current cārnivalé students ONLY

SPB PARTICIPATION PERMISSION
If answering YES - I understand and give permission for my child to participate in the end of year student showcase production.

If answering NO - then please put N/A in the following fields and submit your form by clicking the "submit" button below

SHOWCASE ANNOUNCEMENT EMAIL & INFO SHEET
I have read the showcase announcement email + the SPB information sheet and acknowledge the information contained within and will comply with the T&Cs.

T&Cs SPB CALENDAR/SCHEDULE REQUIREMENTS
I understand that this is a one-month block production schedule (beginning Saturday 14th November and ending on Sunday 13th December) and by enrolling in the SPB I am committing to the showcase performance/s and my child will need to continue to REGULARLY attend all of their normal syllabus lessons and ANY additional rehearsals, dress rehearsals/photography sessions in the month leading up to and on the day/days of their showcase performance/s.

MY CHILD'S PERFORMANCE COSTUME/S HIRE
I understand that my child will require a costume for each dance routine that they are in, and that a costume will be hired to me at a flat rate of $35 per dance routine; this fee is non-refundable, it is per dance and this fee will be added to my T4 fees and payable by WEEK 5.

HIRING OF 2+ COSTUME/S OF THE SAME GENRE
I understand that if my child is in a second routine of the same genre, that this costume will be hired to me at a 20% discounted flat rate ($28 per dance routine); this fee is non-refundable and this fee will be added to my T4 fees.

T&C'S OF PERFORMANCE COSTUME HIRE
I understand that I am "hiring" each costume/prop/accessory (whichever is applicable) and I am solely responsible for any costumes and/or prop/accessories that I am given - any damage and/or loss to any costume/prop/accessory will result in being liable for a full replacement fee of said article.

SOURCING MY CHILD'S PERFORMANCE COSTUME IN WHOLE OR IN PART
I understand that I may be responsible for sourcing my child’s costume in whole or in part, and if so, I will be given a costume brief in WEEK 5 of Term 4.

SPB $10 ADMIN FEE​
I understand there is a $10 flat rate showcase production administration fee. 

SPB ADDITIONAL COSTS
I understand that Term 4 is normally a 9 week term; ALL showcase participating students will be charged as a 10 week term (this ONE extra week’s fees will cover ALL of my child's additional rehearsal costs leading up to the showcase) and this extra week will be added to my T4 fees (payable by WEEK 5); I also know I will be invoiced in WEEK 3 of Term 4 and full payment is required by WEEK 5 before showcase tickets go on sale the following week.

SPB PHOTOGRAPHS
I understand that my child will be photographed individually AND in their class group, during the rehearsals and performance/s for archival and promotion purposes - these photos will be available for purchase

SPB DVD/USB/DOWNLOAD KEEPSAKE
I understand that filming of the showcase on ANY device is STRICTLY PROHIBITED and there will be a keepsake option available for purchase and this service is provided by an independent contractor.
